For certified divers NOT participating in a course
We do not need to collect your medical information. You do not need to print or sign the form. However, please a look at the form to make sure your medical information has not changed significantly since your last dive. The form now includes a question about whether or not you have had COVID-19. It is possible that someone who recovered has lung scarring that could affect their ability to dive comfortably.
For First Time Divers (Try/Basic/Intro Dives)
You must fill out the form.
- If you answered NO to all the questions, you’re done! You can print it and fill it out now, submit it electronically.
- If you answered YES to any question, you must print the form and get a doctor to sign off for diving. It is important because if you have a YES, and then show up unable to dive because of that answer, we cannot provide a refund. We provide refunds outside of the cancellation window (normally 48 hours prior).
For divers participating in ANY scuba diving course
Yes, you have to fill out the form. Open Water Scuba Diver course or Advanced Wreck Diving: you must do this for EVERY course.
- If you answered NO to all the questions, you’re done! You can print it and fill it out now, submit it electronically.
- If you answered YES to any question, you must print the form and get a doctor to sign off for diving. Dr. Young may be able to clear you with a telehealth visit. As of this writing, it was a $90 fee and $130 if you need an in-person visit. If this is something you need, you have to arrange it directly with Dr. Young’s office. His information is only here as a convenience.
